If UCSF IT says your account has been de-activated due to inactivity or expiration, please submit an Account Request Form (ARF) by following the instructions in section #3 (the next slide) of this tutorial:

With this approval, we will get an IT ticket number for you to follow up with UCSF IT and expedite the ticket to be completed as soon as possible / 24 hours.
